From there, identify the information required, the person responsible for it and the moment at which it becomes reliable. The process and the data need to be designed together.

A business does not need to replace every system before improving visibility. Connecting a few reliable sources around a clearly defined operating question can be a practical first move.
The aim is a management view the organisation trusts: what is happening, where it is breaking, who owns the response and whether the change is working. That is a foundation for growth the team can actually manage.
